Abstract

The Family Chart project is a re-imagined patient portal designed to streamline the management of healthcare for all, but especially elderly individuals. Namely, those with chronic conditions and those with caretakers. Family Chart prioritizes ease of use in facilitating communication and coordination among patients, caregivers, and healthcare providers. It simplifies overseeing complex healthcare needs with a focus on user-friendliness and prioritizing easy use. Family Chart simplifies the cumbersome nature often seen in patient portals commonly utilized by healthcare systems by creating a simple management system for appointments, medications, and communication with physicians. Further, Family Chart offers medication reminders, appointment calendars, and secure messaging. The platform is designed to accommodate the needs of all users despite technical acumen and age. By reimagining the common patient portal, Family Chart seeks to reduce the complexities of caregiving to enhance positive health outcomes and foster improved collaboration between all involved in the care process.
